Medicaid Services

Reliable, Local In-Home Support for Medicaid Clients and Their Families

If you are seeking care for an elderly loved one or a loved one with a disability, you may be eligible for a Medicaid waiver. Medicaid waivers allow qualified individuals to receive care services in their homes rather than being placed in a nursing facility. To qualify for a Medicaid waiver in the State of Virginia, you must meet a list of eligibility requirements.

Eligibility:

To determine whether you or your loved one qualifies for a Medicaid waiver, contact your local Department of Family Services Office to begin the eligibility process. The following is an overview of the Medicaid waiver process for new recipients:

  • To be eligible for a Medicaid waiver, a Medicaid Long Term Care Screening is required to assess the functional capacity of the individual.
  • For those who are not receiving Medicaid benefits, an Application for Adult Medical Assistance will need to be completed and sent to the care recipient’s local Department of Social Services office.
  • Once both financial and functional eligibility have been determined, an in-home assessment will be scheduled with a social worker and nurse from the Virginia Department of Health.
  • An approval decision is made within 45 calendar days of the in-home and financial assessment.
  • Once you have been approved for the waiver, One on One Care can start services for you right away. Have your social worker send your paperwork to us, and we will contact you to schedule an in-home assessment to determine which services are right for you.

At One on One Care, we are registered as a provider for:

  • Personal Care and Respite Care (CCC+ Waiver)
  • Developmental Disabilities (DD Waiver)

Do You Want to Change Your Provider Agency?

Are you receiving in-home care via a Medicaid waiver and want to transfer your care to another in-home care agency? You have the right to use any agency you wish. Transferring your services to our agency is simple. Contact your current agency, alert them that you would like to transfer your care to One on One Care and ask them to send us the documents needed for your transfer.
