Expert, Compassionate Support for Seniors With Unique or Complex Needs

When a loved one is living with Alzheimer’s, dementia, Parkinson’s, mobility challenges, or other chronic conditions, finding care you can trust becomes even more important. Our Specialized Care services provide skilled, compassionate support tailored to each individual’s unique needs — helping your loved one remain safe, supported, and comfortable at home.

At One on One Care, you’re not dealing with a national franchise or a corporate playbook. You’re working with a local, family-owned company that has served Central and Northern Virginia since 1998. Our founder, Charles Murray, believed deeply in helping people live better lives at home, and his commitment to individualized, human-centered care still guides every member of our team today.

What Specialized Care Includes

We provide advanced, customized support for seniors with medical, cognitive, or mobility challenges. Depending on your loved one’s needs, specialized care may include:

  • Alzheimer’s and dementia care
  • Parkinson’s and neurological condition support
  • Stroke recovery assistance
  • Chronic illness management support
  • Mobility, transfers, and fall prevention
  • Medication reminders and routine oversight
  • Behavior redirection and safety monitoring
  • Cognitive stimulation and structured daily routines
  • Meal prep, hydration reminders, and nutritional support
  • Personal care assistance tailored to physical limitations

Every care plan is built specifically for your loved one — no templates, no one-size-fits-all approach.

Why Specialized Care Matters

Seniors facing cognitive or physical challenges require more than basic support. They need consistency, patience, and expert guidance from caregivers who understand how to manage symptoms, encourage independence, and promote emotional well-being.

Specialized care helps your loved one:

  • Stay safe and supported at home
  • Maintain routines that improve stability and comfort
  • Reduce confusion, anxiety, and behavioral challenges
  • Prevent falls, wandering, and household risks
  • Stay socially and mentally engaged
  • Enjoy a higher quality of life
  • Remain in familiar surroundings instead of relocating

For families, specialized care brings peace of mind knowing that your loved one is being cared for by trained professionals who truly understand their condition.

Why Families Choose One on One Care

Medicaid Services

Medicaid Services

If you are seeking care for an elderly loved one or a loved one with a disability, you may be eligible for a Medicaid waiver. Medicaid waivers allow qualified individuals to receive care services in their homes rather than being placed in a nursing facility. To qualify for a Medicaid waiver in the State of Virginia, you must meet a list of eligibility requirements.

Eligibility:

To determine whether you or your loved one qualifies for a Medicaid waiver, contact your local Department of Family Services Office to begin the eligibility process. The following is an overview of the Medicaid waiver process for new recipients:

  • To be eligible for a Medicaid waiver, a Medicaid Long Term Care Screening is required to assess the functional capacity of the individual.
  • For those who are not receiving Medicaid benefits, an Application for Adult Medical Assistance will need to be completed and sent to the care recipient’s local Department of Social Services office.
  • Once both financial and functional eligibility have been determined, an in-home assessment will be scheduled with a social worker and nurse from the Virginia Department of Health.
  • An approval decision is made within 45 calendar days of the in-home and financial assessment.
  • Once you have been approved for the waiver, One on One Care can start services for you right away. Have your social worker send your paperwork to us, and we will contact you to schedule an in-home assessment to determine which services are right for you.

At One on One Care, we are registered as a provider for:

  • Personal Care and Respite Care (CCC+ Waiver)
  • Developmental Disabilities (DD Waiver)

Do You Want to Change Your Provider Agency?

Are you receiving in-home care via a Medicaid waiver and want to transfer your care to another in-home care agency? You have the right to use any agency you wish. Transferring your services to our agency is simple. Contact your current agency, alert them that you would like to transfer your care to One on One Care and ask them to send us the documents needed for your transfer.

Please contact us at 540-288-1300 if you have any questions about the care transfer process.
